Rocket Team Nurse

An opportunity has arisen for a permanent full time position with the Respiratory Outreach for COPD in Kettering ( ROCKET Team ) . This band 6 post will provide the successful candidate with excellent experience in Chronic Lung Disease management in both primary and secondary care .

The team specialises in supporting and care planning for individuals with COPD aiming to improve quality of life for those diagnosed, providing alternatives to secondary care admission and ensuring that patients , often complex , clinical needs are assessed and managed.

The team work closely with the wider respiratory service and the emergency / Urgent care departments , identifying suitable patients for supported discharge or admission to the Respiratory Virtual Ward.

The ROCKET Team is at the forefront of developing and implementing the Respiratory Virtual Ward at KGH . The successful applicant will play an active role in the assessment and onboarding of suitable patients and delivering care to those individuals being monitored at home.

The position affords opportunities for career development education , participation in clinical audit / research and pre-registration student mentoring & teaching as well as contributing to COPD patient support groups .
